The aim of the Reablement Team is to ensure service users can remain independent in their own home without any formal support. The team and service will be part of the wider Adult Health & Social Care multidisciplinary teams, which includes direct input from occupational therapists, physiotherapists, and social workers amongst others.
This is a really exciting time to join this new team and contribute to setting up ways of working that forms a key part of Social Care intervention by delivering outcome focused support.
As a Reablement Practitioner you will visit service users in their homes to provide them with support and encouragement to regain their independence.
